Preparing for Your First Appointment

 

Pre-Appointment Checklist

  • Confirm date, time, and physician for your visit.
  • Transfer medical records to your cancer care clinic.
  • Review Parking & Directions.
  • Read Patient Guide [PDF]

What to expect on the day of your appointment

  • Arrive at least 20 minutes prior to scheduled appointment. This allows plenty of time to park, find your clinic, and begin filling out the necessary paperwork.
  • When you check in at your cancer care clinic, a Patient Services Representative will verify your insurance information and ask you to complete new patient paperwork.  
  • Bring a government-issued ID and insurance card to your first appointment. 
  • A medical assistant or nurse will bring you back to the treatment room to begin your appointment.
  • If you have any questions during your visit, please let us know. We are here to assist you.

Post-Appointment Checklist

  • Sign up for an online patient portal account.  This tool will allow you to communicate with your clinic at your convenience.  You can ask questions, request medication refills, access your medical records, make appointments, and more.
  • Schedule a New Patient Orientation by calling Chelsea at (405) 271-8384 to learn more about patient programs and services.
